Rialto has a simple Public Works concern form for local maintenance issues. Use that form when the problem is about a city-maintained place or public area, rather than a private-property question.

The important part is the location. Rialto has long north-south streets, neighborhood edges, school routes, industrial pockets, and older commercial stretches, so “near my house” may not be enough. Add the address if you have it, or the nearest cross street and a landmark if you do not.

Keep the description plain. Say what is wrong, where it is, and whether it blocks a sidewalk, street, driveway, storm drain, sign, or other public feature. A photo can help if the form allows it, especially for a problem that is hard to spot from a moving truck.

If the issue is urgent or unsafe right now, use the emergency or direct city contact path instead of waiting on an online form.
